He's a star of screens both big and small, but Patrick Dempsey has spent enough time in obscurity to never take the success for granted.

The Grey's Anatomy star might be scorching hot in more than 60 countries now, but if he's as anywhere near as ego-obsessed and as full of himself as some of his TV and movie contemporaries, he's doing a really good job of hiding it.

In almost every interview he's done since Grey's Anatomy became a global hit three years ago, Patrick Dempsey has presented himself as genuinely thankful for the role of stethoscope-swinging sex symbol Dr. Derek "McDreamy" Shepherd.

Before Grey's Anatomy came along, Dempsey was wallowing in mediocrity, and plagued by self-doubt. According to the Melbourne Herald-Sun, he actually kept telling himself, depressingly, "I'm ugly, I'm no good, I'm a has-been."

The actor's spirits flagged when he auditioned for, but barely missed out, on a role in the current FOX hit House. Dempsey was called back as many as 15 times by producers who were divided over his suitability for the role.

Patrick Dempsey tested for the role of Grey's Anatomy's Derek Shepherd only after Rob Lowe (The West Wing, Brothers & Sisters) turned it down.

Dempsey also had to contend with dyslexia when trying to read script pages at audition. He tried to memorize lines because he struggled to read them off the page. He couldn't focus and, at certain times, he could hardly speak.

Patrick was convinced that Grey's Anatomy creator Shonda Rhimes, sitting quietly at the back of the audition room, hated him.

Before Michelle Monaghan went public with news that she's pregnant with her first child, Patrick Dempsey was already aware of her condition.

"It was a big secret, and I wasn't allowed to tell anyone, but I knew Michelle was pregnant, and I am thrilled for her," the Grey's Anatomy star said.

"Michelle will be a great mom because she has a good head on her shoulders, a fine husband, and overall, she is a lovely woman."

Patrick, who with wife Jillian Dempsey has 18-month-old sons Darby and Sullivan, and 6-year-old daughter Talula, spoke from experience and gave his Made of Honor co-star some advice on being a first-time parent.

The respective stars of Ghost Whisperer and Grey's Anatomy appear on two alternative covers of the magazine's "Sexiest Stars" issue.

Also making the hot list was another handsome Grey's Anatomy star, Justin Chambers, along with other celebs such as Vanessa Williams, Private Practice leading man Taye Diggs, Eva Longoria and Josh Holloway.

The man known as Dr. McDreamy may not know how to make a marriage last on Grey's Anatomy â€" but the man known as Patrick Dempsey sure does!

"Love, humor, communication. To me those are the bedrocks of a good relationship," the Versace-clad Dempsey, 42, said Monday at the New York City premiere of his wedding-themed romantic comedy, Made of Honor.

The Grey's Anatomy star plays a compulsive womanizer in the comedy, but says the Made of Honor character's "nothing like me at all â€" I'm married and have kids!"

Of his wife, Jillian Dempsey, the appreciative father-of-three said:

"She's immensely supportive. Without her, I wouldn't be here."

They have a six-year-old daughter and one-year-old twin sons.

So does Patrick Dempsey â€" who was recently featured in USA Today among other publications â€" have any funny memories from their 1999 nuptials?

"The photographers were drunk and never got a good picture!" he joked.

During his writers'-strike-induced hiatus from Grey's Anatomy, Patrick Dempsey whisked his wife, Jillian Dempsey, away for a romantic Italian getaway.

No kids. No cameras. And no reporters.

The couple took in a fashion show in Milan; then, in Lake Como, they were among many marveling at the estate of a famous resident: George Clooney.

"He's larger than life," Patrick Dempsey told USA Today. "I've met him a few times now, and he has always been very funny and charming. I like him."

Although he'd perhaps prefer comparisons with his idol, Cary Grant, it is Clooney with whom Patrick Dempsey has been compared since he rejuvenated a stalled teeny-bopper career by hitting it big on a prime-time medical series.

With his comedy Made of Honor hitting theaters May 2 and potentially solidifying his movie stardom for the first time since his late '80s hits â€" Can't Buy Me Love (as a dorky kid who hires a cheerleader to be his girlfriend) and Loverboy (as a pizza delivery boy who delivered a little something extra) â€" Clooney comparisons are back.

Patrick Dempsey has plenty to smile about these days.

Dempsey, 42, got an early jump on his leading-man status last year by wooing princess Amy Adams away from the charming James Marsden in Enchanted. Its $128 million take has producers dreaming up a sequel. But Enchanted was an ensemble.

"This is the first time I've carried a movie in a long time," Dempsey says over drinks at a corner table in the Four Seasons Hotel bar. As a result, he has been asking himself, "Can I pull this off? Will people come see it?"

"This is a very big film for him," says Made of Honor director Paul Weiland. "Patrick grew up in the (Hollywood) system, but I'm not sure he wants to stay in it forever."

Dempsey concedes that he "wasn't sure if Enchanted was going to work." So he sought a project that would be "light and different from Grey's Anatomy. With a broad-based appeal. Something that would do well commercially to allow me to cross over."
