I doubt Derek would over-react like that (about Mark and Lexie). Remember Derek didn't even really want to caution Mark about Lexie. He only asked Mark to leave Lexie alone because Meredith was concerned for Lexie a while back and didn't want Mark to complicate things for her. I think it's got to be something more to start a fist fight.

I think it's cuz he finds out that Addison was pregnant with Mark's child and she had an abortion

I dunno, I think it may be that Derek and Mark's showdown happens after that surgery scene, and Derek is still freaking out and Mark decides to have really bad timing, and Derek just freaks. I mean he is proposing now, so maybe he has a different view (more like Mer's) on a relationship between Mark and Lexi....wow, though, right? this is gonna be really freaking intense! eeeek!!!

Or maybe Derek founds about Mark and GREY, and punch before knows it's little Grey! With all the patient stress in his mind, and thinking about losing Meredith... I guess only this could make him react so badly...

OMG this looks so awesome! It's like the writers have woken up from a half-season long coma. I was worried in the beginning of this season, but these moral dilemmas and real relationships are what I love Grey's for. This totally brightens my Thursday ^.^

I think Derek has been getting increasingly annoyed and maybe now even angry about Mark blurting out to everybody that he is going to propose to Meredith. However, Mark only told Addison to cover his a.. so that Derek wouldn't know about him and Lexie. It can't only be Lexie related.
I feel that Mark and Derek's friendship isn't really that solid and it is definitely not balanced. Mark shouldn't have to worry about his friend's approval of a girlfriend. It will be interesting to see how it plays out.

I think saira has an interesting angle. I had forgotten about the abortion and it did cause Addison some pain and the fact that neither of them ever told Derek...it could cause Derek to snap especially if it is just after the operation on Jen. I think it will definitely be something from the past and not to do with Mark and Lexie
