Mer is terrified of letting Der love her because she feels she's not worthy of love, she's a failure (she's ashamed of herself/ her past) - she doens't believe that she deserves good things in life; she's afraid that Der will one day see right thru her and get sick of her skeletons/demons and just like Burke didn't choose Cris Der will get tired of making up for her emotional shortcomings (breathing for her) - She will never be a perfect wife, lover, mother, a nurturer (far from Addison or happily ever after).
She has to regaiin the spark from S1 where she was fearless, focused, inspired, so un-ordinary, self-confident despite her limitations/fears. She deserves to have a family (even if it's Lexi and a father figure in the Chief); she deserves to be loved by a good man (even if it's a man with baggage like Der); she deserves to be focused on her job (pick a specialty); she deserves to have fun, smile a little enjoy her friends and get her mojo back. And most of all she deserves to be happy she needs to WANT all those things and fight for them....
The good news is there's nothing that Der doesn't know that will scare him away from her (he has seen her thru the worst most embarrasing, heartbreaking, suicidal moments in her life) she just needs to let him in and maybe she'll see just how damaged and fragile he is as well so she doesn't have to feel like a failure compared to him)
